ISO 9001 Certification in Australia: Why Most Businesses Fail the Stage 2 Audit | S& J Auditing & Consulting
Most businesses that fail their ISO 9001 Stage 2 audit don't fail because their processes were wrong. They fail because their documented information didn't reflect what their processes actually were.
Clause 7.5 requires documented information that supports the operation of your QMS not a policy folder that was accurate eighteen months ago. The gap between what's written and what's happening on the ground is the single most common audit finding.
ISO 9001:2015 certification runs on a 3-year recertification cycle, with annual surveillance audits in between. Organisations that treat those as administrative checkpoints, rather than system reviews, tend to accumulate nonconformities quietly — until the recertification audit surfaces them all at once.
The practical question before Stage 1 isn't "are we ISO 9001 ready?" It's "can we demonstrate conformance against every applicable clause, with documented evidence, to an auditor we've never met?"
If that question creates hesitation, the gap analysis should happen before the audit booking.
